In an email including your full name and the name and species of your pet, attach a minimum of two good digital snapshots of your pet (preferably in different poses/from different angles), and preferably several photos, especially if you are ordering a Keepsake Book. Email photos to photos@petpoetraits.com. If possible, please include one of you together (this is strictly to give us a visual of the bond between you. Your image will not be used in the artwork). Then return to this page and proceed to Step 2.
Please answer the following questions to the best of your ability, so that we can capture your pet’s personality and the essence of your special relationship. Don’t share anything unless you want it incorporated in the poem, but other than that, no information is too much information. The more you share with us, the better and more personal to you our work will be. (We recommend that before you submit this form, you copy and paste it into a document you can save, so that if for any reason the submission fails, you won’t lose your answers.)
